Form 1


Certain public officers and employees, including certain university employees, must file a Statement of Financial Interests (Form 1) with the Florida Commission on Ethics. This is required by Florida’s ethics laws (112.3144 and 112.3145, F.S.) and enables the public to evaluate potential conflicts of interest, deters corruption, and increases public confidence in government.

Frequently Asked Questions

UFCE will notify employees to whom the Form 1 filing requirement applies. Reporting individuals or "filers" at UF generally include:  

  • Board of Trustees 
  • President 
  • Chief Compliance, Ethics, and Privacy Officer 
  • Vice President of Human Resources (Personnel Officer) 
  • Deans 
  • Attorneys 
  • Procurement Agents  
  • Employees with purchasing authority that exceeds $35,000 
  • Individuals on the Delegation Signature Authority Chart  
  • Select Grant Coordinators  

Filers are required to submit Form 1 to the Florida Commission on Ethics annually by July 1 each year.  Employees that fail to timely file their Form 1 could incur fines.  

Filers must also file an Initial Form 1 within 30 days of being hired/ moving into a filer position, and a Final Form 1F within 60 days of separating from a filer position.  

Filers must use the Electronic Financial Disclosure Management System (EFDMS) to file Form 1 with the Florida Commission on Ethics. Paper forms are not accepted.  

  • UFCE will notify filers when they are added to the filing list, and the Florida Commission on Ethics will send an automated email with instructions on creating an account.  
  • Create a user account in EFDMS using your UF email.  
  • File your Form 1 financial disclosure in EFDMS.

Form 1 is due July 1 of each year for the preceding calendar year. It is considered late if filed after July 1.

If the disclosure is not filed by September 1, an automatic fine of $25 per day will begin to accrue until the disclosure is filed, or the fine reaches $1,500.   

  • If you have received a fine, you will receive an email from the Commission to the email address that is displayed on your profile in the EFDMS system. The email will include the form year, the amount of your fine, and payment options. Your name, organization and fine amount will also be displayed on the public fines accrued report for the relevant form year. 
  • Failure to file can result in removal from office or employment. 

If a filer is unable to meet the annual filing deadline, they may request an extension through their EFDMS user profile. The Commission will review the request and either approve or deny it. If approved, a new filing deadline will be assigned.   

Any previously filed Form 1 may be corrected or amended via the use of Form 1X, Amendment to Statement of Financial Interests, found on the Commission's forms page. The form is filed electronically in EFDMS.
